Section 01
Acceptance of Terms
By accessing or using the Meadow Health website (meadowhealth.org) or any of our services, you agree to be bound by these Terms of Service ("Terms") and our Privacy Policy. If you do not agree to these Terms, please do not use our services.
These Terms constitute a legally binding agreement between you and Meadow Health, LLC, a limited liability company registered in the State of Oregon.

Our Services
Meadow Health provides a telehealth platform focused on PCOS and women's hormonal health. Our services include:
- Access to licensed clinical providers specializing in PCOS and hormonal conditions via telehealth
- Personalized care plans developed by licensed clinicians
- Coordination of lab testing including hormone panels and DUTCH testing
- Supplement recommendations and access to a curated supplement dispensary
- Ongoing provider messaging and monthly check-in appointments
- Educational content, articles, and community resources
Clinical services are provided through our licensed clinical infrastructure partner. Meadow Health, LLC serves as the platform connecting members with clinical care — we are not a medical practice.
Section 03
Eligibility
To use Meadow's services, you must:
- Be at least 18 years of age
- Be a resident of a state in which our clinical services are available
- Provide accurate and complete information during registration and intake
- Not use the platform for any unlawful purpose
By registering, you represent and warrant that you meet all eligibility requirements listed above.
Section 04
Membership & Billing
Subscription terms
Meadow membership is offered as a monthly subscription at the price displayed at the time of enrollment. Your subscription will automatically renew each month unless you cancel prior to your renewal date.
Payment
Payment is processed securely through Stripe. By providing your payment information, you authorize Meadow Health, LLC to charge your payment method on a recurring monthly basis until you cancel.
Cancellation
You may cancel your membership at any time through your account settings or by contacting our support team. Cancellation takes effect at the end of your current billing period. We do not offer partial-month refunds.
Refunds
Refund requests are evaluated on a case-by-case basis. If you are unsatisfied with your first appointment, please contact us within 7 days and we will work with you to find a resolution.

Clinical Services

This section covers the relationship between Meadow as a platform and the licensed clinical providers delivering care. The specific legal structure (Meadow as a management services organization vs. a direct provider platform) must be confirmed with a healthcare attorney to ensure compliance with corporate practice of medicine laws in Oregon and all states served.
Clinical services available through Meadow are provided by licensed healthcare professionals through our clinical infrastructure partner. These providers are independently licensed and exercise their independent professional judgment in all clinical decisions.
Meadow Health, LLC does not practice medicine and does not employ the licensed providers who deliver care through our platform. The clinical relationship is between you and your licensed provider.
You understand and agree that:
- Clinical recommendations are made at the sole discretion of your licensed provider
- Meadow cannot guarantee specific clinical outcomes
- You are responsible for following your provider's recommendations and disclosing all relevant health information accurately
- Telehealth services have inherent limitations compared to in-person care
Section 06
Not Emergency Care
Meadow is not an emergency medical service. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.
Meadow's services are not appropriate for acute or emergency medical conditions. Our platform is designed for ongoing management of chronic conditions including PCOS, hormonal imbalance, and related metabolic concerns.
Section 07
User Conduct
By using Meadow, you agree not to:
- Provide false or misleading health or personal information
- Use the platform for any unlawful or fraudulent purpose
- Attempt to access other users' accounts or information
- Harass, threaten, or harm any provider or Meadow staff
- Reproduce, distribute, or commercially exploit any content from our platform without written permission
- Attempt to interfere with the security or functionality of our platform
Meadow reserves the right to suspend or terminate accounts that violate these conduct standards.

Intellectual Property
All content on the Meadow platform — including text, graphics, logos, quiz content, articles, and design — is the property of Meadow Health, LLC or its content suppliers and is protected by applicable intellectual property laws.
You may not reproduce, distribute, or create derivative works from our content without our express written permission. Personal, non-commercial use of our educational content is permitted with appropriate attribution.

Disclaimers
Meadow's platform and content are provided "as is" without warranties of any kind, express or implied. We do not warrant that our services will be uninterrupted, error-free, or that any particular health outcome will be achieved.
Educational content on our website — including articles, blog posts, and quiz results — is for informational purposes only and does not constitute medical advice. Always consult a licensed healthcare provider before making decisions about your health.
Meadow Health, LLC does not guarantee the accuracy, completeness, or timeliness of any information on our platform.
Section 10
Limitation of Liability

To the maximum extent permitted by applicable law, Meadow Health, LLC and its officers, members, employees, and partners shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of your use of or inability to use our services.
Our total liability to you for any claim arising out of or relating to these Terms or our services shall not exceed the amount you paid to Meadow in the three months preceding the claim.
Section 11
Governing Law & Dispute Resolution
These Terms are governed by and construed in accordance with the laws of the State of Oregon, without regard to its conflict of law provisions.
Any disputes arising out of or relating to these Terms or our services shall first be attempted to be resolved through good-faith negotiation. If negotiation fails, disputes shall be resolved through binding arbitration in accordance with the rules of the American Arbitration Association, conducted in Oregon.
