By using Meadow Health's services you agree to these Terms. Meadow provides clinical telehealth services — not emergency care. If you are experiencing a medical emergency, call 911 immediately.
Meadow Health, LLC ("Meadow") is a telehealth platform based in Oregon that connects patients with licensed Nurse Practitioners focused on PCOS and metabolic health. Meadow is not a hospital or emergency service.
Meadow reserves the right to decline or discontinue service where care falls outside our clinical scope, or where treatment would not be safe or appropriate.
When enrollment opens, clinical care will be provided through appropriately qualified and licensed nurse practitioners. All clinical decisions — including diagnoses, treatment plans, and prescriptions — are made solely by your licensed provider. Meadow does not practice medicine and does not direct clinical decisions. No guarantee of specific outcomes is made.
Not every patient is a candidate for every treatment. Whether medication is appropriate for you is determined by your provider after review of your history and laboratory results.
Meadow is preparing the following program; enrollment and payments are not open:
No payment authorization is being collected. Final payment terms will be provided before enrollment.
Laboratory and specialty testing, prescription medications, supplements, and devices or other third-party products and services are paid separately and are not included in Meadow's fees unless Meadow explicitly states otherwise.
Final cancellation and refund terms for the six-month program are pending review and will be supplied before enrollment. No payment is being collected.
Meadow is not an emergency medical service. Do not use Meadow for medical emergencies. Call 911 or go to your nearest emergency room immediately in any emergency situation.
To the fullest extent permitted by law, Meadow Health, LLC and its members, managers, employees, and contractors shall not be liable for any indirect, incidental, or consequential damages. Meadow's total liability shall not exceed the amount you paid to Meadow in the twelve months preceding the claim.
All content on meadowhealth.org — including text, protocols, graphics, and branding — is the property of Meadow Health, LLC. You may not reproduce or distribute our content without prior written permission.
These Terms are governed by the laws of the State of Oregon. Disputes shall be resolved through binding arbitration in Portland, Oregon under the rules of the American Arbitration Association.
We may update these Terms from time to time. Material changes will be posted on this page with a revised date. Continued use of our services constitutes acceptance.
