By using Meadow Health's services you agree to these Terms. Meadow provides clinical telehealth services — not emergency care. If you are experiencing a medical emergency, call 911 immediately.

1. About Meadow Health

Meadow Health, LLC ("Meadow") is a telehealth platform based in Oregon that connects patients with licensed Nurse Practitioners specializing in PCOS and women's hormonal health. Meadow is not a hospital or emergency service.

3. Clinical Services

Meadow provides telehealth clinical services through licensed Nurse Practitioners. All clinical decisions — including diagnoses, treatment plans, and prescriptions — are made solely by your licensed provider. Meadow does not practice medicine and does not direct clinical decisions. No guarantee of specific outcomes is made.

4. Membership & Billing

Meadow memberships include a one-time enrollment fee and a recurring monthly membership fee. By enrolling you authorize Meadow to charge your payment method on a recurring basis. Lab testing and prescription medications are billed separately and are not included in your membership fee.

5. Cancellation & Refunds

You may cancel your membership at any time with no cancellation fee. Your membership remains active through the end of your current billing period.

First-visit guarantee: if you are not satisfied with your first provider appointment, contact us at hello@meadowhealth.org within 7 days and we will refund your first month's membership fee. The enrollment fee is non-refundable after your first appointment is completed.

7. Not Emergency Care

Meadow is not an emergency medical service. Do not use Meadow for medical emergencies. Call 911 or go to your nearest emergency room immediately in any emergency situation.

8. Limitation of Liability

To the fullest extent permitted by law, Meadow Health, LLC and its members, managers, employees, and contractors shall not be liable for any indirect, incidental, or consequential damages. Meadow's total liability shall not exceed the amount you paid to Meadow in the three months preceding the claim.

9. Intellectual Property

All content on meadowhealth.org — including text, protocols, graphics, and branding — is the property of Meadow Health, LLC. You may not reproduce or distribute our content without prior written permission.

10. Governing Law

These Terms are governed by the laws of the State of Oregon. Disputes shall be resolved through binding arbitration in Portland, Oregon under the rules of the American Arbitration Association.

11. Changes to These Terms

We may update these Terms from time to time. Material changes will be posted on this page with a revised date. Continued use of our services constitutes acceptance.
