Step 1

We saved your progress from last time.

Let's get started

What is your primary goal with Meadow?

Choose the one that matters most right now.

Your symptoms

Which of these do you experience?

Select all that apply.

Your diagnosis

Where are you with PCOS?

Meadow is a PCOS practice. This helps us know if we are the right fit.

About you

What is your date of birth?

Meadow treats adults 18 and older.

You must be 18 or older to use Meadow.
Your location

Which state will you be in for your appointments?

Our providers are licensed in Oregon and Washington. Telehealth rules follow where you are physically located during the visit.

Safety screening

Are you currently pregnant or breastfeeding?

The medications we may use are not safe during pregnancy or breastfeeding.

Safety screening

Are you currently trying to conceive?

There is no wrong answer. This changes what kind of care is right for you.

Safety screening

Are you willing to use contraception while in treatment?

As your metabolism improves you may start ovulating again, sometimes unexpectedly. Because these medications must be stopped before pregnancy, contraception is required while you are on them. You come off well before you start trying.

Your current care

Are you currently taking a GLP-1 medication?

Such as Wegovy, Ozempic, Zepbound, Mounjaro, or a compounded version.

Medical history

Do any of these apply to you?

Select all that apply. If any do, a nurse practitioner will review your situation personally before anything moves forward.

What we are hearing

You are not imagining this.

Here is what you told us.

Your symptoms

That combination is a pattern, not a personal failing. It usually points to something specific happening underneath, and that something is measurable. We believe we can help.

    How Meadow works

We treat the cause, not the symptom.

Most PCOS care manages symptoms one at a time. We start by finding what is driving them.

    1
Your goalsWe start with what you actually want to change.
    2
Your labsPCOS is not one condition. Bloodwork shows us which pattern is driving yours.
    3
Your planA protocol built around your results, with a realistic timeline and a defined end.
    4
Your providerA licensed nurse practitioner who works with PCOS every day.
    One thing to confirm

Meadow requires bloodwork before any treatment.

Labs are ordered at signup and paid separately, directly to the lab. We do not prescribe without them. This is the part that makes everything else work.

    One quick thing

    How did you find Meadow?

This helps us know where to show up for women like you.

    Tell us more

Anything else you want us to know?

What have you already tried? What has been most frustrating? Optional, but your provider will read it before your first appointment.

    Almost there

We are not in your state yet.

Our providers are licensed in Oregon and Washington only, so we cannot take you on as a patient today.

We are expanding. Leave your email and we will tell you the moment Meadow opens where you live.

    🌿
    Thank you.

We have your email and we will be in touch.

    🌿
A fertility specialist is the right next step.

Because you are actively trying to conceive, Meadow is not the right fit right now. The medications we work with have to be stopped well before pregnancy, so starting them now would work against what you want most.

A reproductive endocrinologist can help you directly, and can address PCOS as part of that care.

Oregon Fertility Institute Dr. Aimee Chang, board certified in reproductive endocrinology and OB-GYN, with a stated focus on PCOS. Tigard, Oregon.

If your plans change, or if you decide you want your metabolic health sorted out before you start trying, we would love to hear from you.

    🌿
Congratulations. We will be here when you are ready.

The medications we work with are not safe during pregnancy or breastfeeding, so we cannot enroll you right now.

Stay with your OB or primary care provider for now. When you are ready to focus on your metabolic health, come back to us.

    🌿
We want a clinician to look at this with you.

Thank you for telling us. Based on what you shared, a nurse practitioner needs to review your history personally before we can say whether Meadow is a safe fit. That is not a no, it is a conversation.

Leave your email and we will be in touch within one business day. No charge, no obligation.

This is also worth raising with your own doctor, who has your full history.

Please enter a valid email address.
